Participant Information/Emergency Forms
As we did last year, we will utilize ePACT for all information/emergency forms and authorized pick-up person lists. There will be no paper forms available to complete; all information must be submitted through ePACT before the first day of preschool.
If you have already created an ePACT account and submitted information for your preschooler for last school year or for summer camp, you do not need to create a new account or resubmit the form; just please review your information to make sure everything is up-to-date.
If you do not already have an ePACT account, you should have recieved with a link to complete your child's participant information/emergency form. The email will come from email@example.com; the subject line will include "Downers Grove Park District-Early Childhood Programs".
At this time, we do not plan to use to QR code feature for checking students in/out, but it is still essential that you list all authorized pick-up persons on your child's form.Using ePACT Help Sheet
